Eligibility

ONLY Healthcare workers WHO RECEIVED THEIR FIRST VACCINATION AT THE JAN 13 CLINIC may receive the booster shot at this time. If you did not receive your first shot at the Jan 13 clinic, you will not receive a followup shot.

Required Documents

Vaccine recipients are REQUIRED to bring the following documents to your appointment:

 

  • Proof of healthcare worker status (for example, a copy of license or letterhead from healthcare office)
  • Current Insurance Card
  • Driver’s License 
  • Immunization Consent Form 
Immunization Consent Form

A completed consent form is required to receive the Covid vaccination. Please download, print, and complete the attached Immunization Consent Form before proceeding.

 

Your Appointment:

This vaccination event is not open to the public and is restricted to health care workers listed in Phase 1a. Our clinic will adhere to the priority guidelines set forth by the ADH. You can schedule an appointment using the link below. You will need to identify your job category before finalizing your appointment.

We can’t afford no shows so if you are unable to keep your appointment, please cancel.

All recipients will be required to remain in the observation area for 15 minutes after vaccination per CDC guidance. The vaccine will be provided at ZERO cost to the recipient or facility. The pharmacies will bill your insurance or the COVID-19 Uninsured Program for the administration fee.
