Medical Arts Pharmacy is proud to offer COVID-19 vaccinations for our community.

Eligibility
ONLY Healthcare workers may receive a vaccine at this time. If you are not a front-line healthcare worker in Northwest Arkansas, you will not receive a vaccine. Please contact the pharmacy regarding dates for Phase 2-3 vaccinations.
Required Documents
Vaccine recipients are REQUIRED to bring the following documents to your appointment:
- Proof of healthcare worker status (for example, a copy of license or letterhead from healthcare office)
- Current Insurance Card
- Driver’s License
- Immunization Consent Form
Immunization Consent Form
A completed consent form is required to receive the Covid vaccination. Please download, print, and complete the attached Immunization Consent Form before proceeding.

WHEN YOU ARRIVE FOR YOUR APPOINTMENT, YOU WILL ENTER THE DOOR TO THE LEFT OF THE MAIN DOOR (BETWEEN MEDICAL ARTS AND MANA SOUTH CLINIC) SO WE CAN MAINTAIN SOCIAL DISTANCING AND KEEP FOOT TRAFFIC ONE WAY.
