Eligibility

ONLY Northwest Arkansas residents who are 70+ are eligible to receive a 1-B vaccine at this time. If you are not 70+ years old, you will not receive a vaccine. Please contact the pharmacy regarding dates for Phase 2-3 vaccinations.

Required Documents

Vaccine recipients are REQUIRED to bring the following documents to your appointment:

 

  • Proof of age (a drivers license or other photo ID)
  • Current Insurance Card
  • Driver’s License 
  • Immunization Consent Form 
Immunization Consent Form

A completed consent form is required to receive the Covid vaccination. Please download, print, and complete the attached Immunization Consent Form before proceeding.

WHEN YOU ARRIVE FOR YOUR APPOINTMENT, YOU WILL ENTER THE DOOR TO THE LEFT OF THE MAIN DOOR (BETWEEN MEDICAL ARTS AND MANA SOUTH CLINIC) SO WE CAN MAINTAIN SOCIAL DISTANCING AND KEEP FOOT TRAFFIC ONE WAY.
